General

To cancel your policy please contact the Nassau Service Center at 1-800-541-0171. Surrender forms are individually created for each policy by phone only.

For service or claims inquiries on your Pyramid Life or Constitution Life policy, please contact us at 1-800-999-2224. Please note: The Pyramid Life Insurance Company (“Pyramid Life”) and Constitution Life Insurance Company (“Constitution Life”) were renamed Nassau Life Insurance Company of Kansas (NLIC of Kansas) and Nassau Life Insurance Company of Texas (NLIC of Texas), respectively. NLIC of Texas merged into and became a part of Nassau Life and Annuity Company.

Life Insurance

Payments can be made through bank draft or electronic funds transfer (EFT) for monthly mode only. Premium payments can also be made via personal check for quarterly, semi-annual or annual payment modes.

A form is required to set up payment via EFT for the first time or to make updates to pay modes or other information. Visit Service Forms to download the appropriate form.

For certain policy types, we can accept one-time payments only online via EFT. If you are already on monthly bank draft for premium payments you can also update your routing and bank account number online. Please register/log in to access your account to make an one-time online payment or update banking information.

Payments cannot be made via money order or credit card at this time.

Complete the Check-O-Matic service form and submit it to the Nassau Service Center. Visit Service Forms to download the appropriate form.

For certain policy types, if you are already on monthly bank draft for premium payments you can also update your routing and bank account number online. Please Register/log in to access your account and make request updates.

Annuities

There are different ways to withdraw funds from your annuity contract. If you would like to discuss your different options, please contact our Nassau Service Center at 1-800-541-0171. You may also visit Service Forms to download the appropriate form based on state of contract and product type.

Please complete the Annuity Direct Deposit form and make sure to include a voided check for the new account. Visit Service Forms to download the appropriate form.

To activate your income rider complete the Guaranteed Lifetime Withdrawal Rider Activation form. Please note, in most cases, the rider cannot be activated until 30 days from the issue date. This benefit can only be elected at issue. Visit Service Forms to download the appropriate form.

First year required RMDs, should be taken from your existing qualified account prior to transferring the funds to Nassau. An annual notification is sent to contract holders of qualified accounts once they have reached the age of 70 1/2 (this applies if you turned 70 1/2 prior to 12/31/2019). As of 1/1/2020, the qualifying age is 72.

Nassau sends out tax information by the end of January each year. The Service Department is prepared to assist you during tax season. We are able to explain your 1099R’s and send duplicate copies upon request. Form 5498 is sent out in mid-May. Please be advised we do not provide tax advice.
